For the purpose of your present and future treatment, your personal and care details will be documented. This may be on paper or on computer. We use the information held on patients for their benefit eg reviewing and monitoring the patients health and sending for health screening. We may also use the information for other reasons such as research, audit and training purposes. In addition there are certain services provided by the practice that attract payment, which require to be verified by other NHS staff.
